Application forms (for SRL Licence)

Once you have completed the SRL training programme, and passed your Maritime NZ (MNZ) Final Oral Examinations, you will need to apply to MNZ for your SRL licence. All forms and guidelines for SRL can be found on the Maritime New Zealand website www.maritimenz.govt.nz

To receive your SRL licence, MNZ currently require you to:

1. complete your SRL training through SiS – enrol here online
2. complete the MNZ SRL Application form
3. complete the MNZ Fit and Proper Person form
4. supply your First Aid and MRROC certificates
5. supply the required MNZ identification, photos, eyesight report and medical certificate.
